Hours of Operation - Monday - Saturday 10:00 am - 5:00 pm; Sunday 12:00 pm - 5:00 pm d. Business will not target a specific segment of the community e. This business will benefit the pediatric program of Ho Spice of the East Bay f. There will be no disruption to surrounding business g. There will be no negative effects on the health or safely of surrounding business h. There will be no negative impacts on property or transportation systems. i. ATTACHMENT 2 Hospice of the East Bay 3470 Buskirk Avenue • Pleasant Hill, CA 94523 (925) 887-5678 www.hospiceeastbay.org Hospice of the East Bay was founded in 1977 to provide pain contro and symptom management, along with emotional, spiritual, practical, and bereavement support, to terminally ill patients and their families. When a loved one is diagnosed with a terminal illness or condition, it is distressing news for everyone in the family. Multiple surveys report that 85-90% of Americans express the wish to remain secure at home at the end of their lives, surrounded by the people and things that they love, comforted by familiar routines, and in control of their medical and personal options. Our hospice care teams provide that opportunity. Over the years, more than 15,000 families have benefited from the e?,pertise and compassion of our hospice professionals. In addition to adult hospice care, we offer two specialized programs: Comfort for Kids (hospice and palliative care for children 0-18 years of age) end Anna's Program (care for women with recurrent breast cancer). In 2004, we opened Bruns House in Alamo. The only inpatient hospice facility in the East Bay, the House features six private bedrooms, 24-hour medical care for patients and unlimited visiting hours for their family and friends. At Bruns House, patients experience privacy and quiet that cannot be achieved in a hospital, and constant monitoring and treatment that cannot be achieved at home. At Hospice of the East Bay, three teams provide care for adults, and one team is specially trained to care for children. Teams include hospice physicians, and nurse/case managers-all experts at controlling pain and managing other complex symptoms common to terminal illness-as well as medical social workers, home health aides, and spiritual and bereave nent counselors. In addition, 85 trained volunteers provide companionship and assistance with many practical needs of patients and their family caregivers (i.e. driving to appointments, light cooking and housekeeping). In 2007, our clinical teams cared for almost 1600 patients and their family members in their own homes, at Bruns House, or in a variety of skilled nursing or assisted living facilities. Wherever services are provided, we consider the entire family to be the unit of -,are. All medically qualified patients are accepted, regardless of their insu-ance status or ability to pay for services. The Hospice Foundation of the East Bay, formed in 1999, covers the cost of otherwise uncompensated care by soliciting individual and corporate donations, writing grants, sponsoring many fund-raising events, and managing five thrift shops. Our hospice consistently earns Gold Seal Accreditation from JCAH0, the Joint Commission on Accreditation of Healthcare Organizations, which evaluates the quality of care delivered by hospitals, surgery centers, and hospices.
