EXECUTIVE SUMMARY:
Under the direction of the City Council, the Human Services Commission is responsible for
providing Human Services Grants Program funding recommendations to the City Council each
year. On January 28, 2016, the Commission held a public meeting related to the Fiscal Year
2016 -2017 Human Services Grants Program. The Commission received presentations from 17
community organizations that submitted applications for 19 programs. After deliberation, the
Commission recommended 18 programs receive grant funding in the upcoming cycle.
FINANCIAL IMPACT:
The Human Services Grants Program is supported by several funding sources including
Community Development Block Grant (CDBG) funds, the General Fund, and the Affordable
Housing Fund (as appropriate). The City's Fiscal Year 2016 -2017 CDBG allocation is
anticipated to be $70,709 for direct support to social services programs. The General Fund
funding cap is $130,000 and the Commission recommends an allocation of $8,117 from the
Affordable Housing Fund to support housing programs consistent with the intended use of the
fund and past practices.
RECOMMENDATION:
Staff recommends that the City Council adopt the Resolution Approving Funding
Recommendations for Fiscal Year 2016 -2017 Human Services Grants Program, and adopt the
Resolution Approving Funding Recommendation for a Fiscal Year 2016 -2017 Human Services
Grants Program Contribution to Dublin Partnerships in Education.

DESCRIPTION:
The City of Dublin Human Services Grants Program provides financial support to local non - profit
organizations providing social services in the Dublin community. The program is supported
through several funding sources including the Federal Community Development Block Grant
(CDBG) program, the City's General Fund, and the Affordable Housing Fund (as appropriate).
The City Council assigned the responsibility for making grant funding recommendations to the
Human Services Commission. This is the third year the Commission has made funding
recommendations to the City Council.
On January 28, 2016, the Human Services Commission held a public meeting and received
presentations from 17 community organizations for 19 programs (Staff Report as Attachment 1).
The Commission's deliberations resulted in the funding recommendations outlined below, which
are also contained in Attachment 2. Attachment 3 provides details about the grant program
guidelines, public outreach activities, and the process the Human Services Commission
followed in making their funding recommendations.
Analysis
The City's Fiscal Year 2016 -2017 CDBG allocation is anticipated to be $72,709 for direct
support to social services programs. The General Fund funding cap is $130,000 and the
Commission recommends an additional allocation of $8,117 from the Affordable Housing Fund
to support housing programs consistent with the intended use of the fund and past practice
During this year's application cycle, the City of Dublin received 21 applications from 18
organizations. One of these groups, YMCA of the East Bay /Tri- Valley YMCA, submitted
applications for two programs but did not attend the meeting before the Human Services
Commission. Under the rules outlined for the applying organizations, attendance at the
Commission presentation is required in order to receive funding (Attachment 3).
A total of $282,503 was requested through the Human Services Grants Program. Under the
grant guidelines, each organization is eligible for a minimum award of $3,500 and a maximum of
$25,000. The following chart summarizes the total funding requests and Human Services
Commission total recommendations for the FY 2016 -2017 Human Services Grants Program.

CDBG Funds
Eight organizations qualified for and indicated they were able to meet the CDBG regulations.
Six organizations are recommended for funding through the CDBG allocation and the remaining
two organizations are recommended through other funding sources, the General Fund and the
Affordable Housing Fund, in order to maximize funding availability.
The organizations, as recommended, meet the CDBG program's national objectives of
benefitting income - qualified persons, homeless persons, or persons with disabilities, and all
qualify as eligible uses of CDBG funds. If approved, the City would enter into mandatory
contracts with Alameda County and the funded organizations.
The table below summarizes the CDBG funding recommendations, indicating Staff's initial
recommendation and the final recommendation from the Commission. Of the anticipated
$72,709 allocation, $13,374 is earmarked by the City Council for support to Axis Community
Health for the construction of their new health facility in Pleasanton. This agreement was
executed in July 2015.
Axis Community Health submitted an application requesting funds for pre- construction
(planning) costs for development of a dental care facility. This type of project request is not
eligible for CDBG funding as a service or capital project. As a result, Staff recommended that
the Commission consider funding this project using the one -time $30,000 the City Council set
aside for the Commission to fund a one -time special project. The Human Services Commission
agreed with Staff's recommendation and will consider the request at a future Commission
meeting.

The City Council approved a General Fund allocation of $130,000 for the FY 2016 -2017 Human
Services Grant Program. Applicants requested a total of $175,611 from the General Fund.
YMCA of the East Bay/Tri- Valley YMCA did not attend and present at the Human Services
Commission meeting, and consequently the organization did not meet the requirements set forth
in the City guidelines to remain eligible for grant funding. The YMCA had requested funding for

two programs - Mental Health Services ($25,000) and Affordable Summer Child Care
($25,000). Staff recommended each program receive $10,000 ($20,000 total). Because they
weren't present at the meeting, the Human Services Commission reallocated Staff's funding
recommendations for YMCA of the East Bay /Tri- Valley YMCA to other organizations, such as
Dublin Partners in Education (DPIE) and the Dublin High School Parent Faculty Student
Organization (PFSO), and in some cases, recommended amounts in excess of the
organization's original funding requests (see table below).
Staff is concerned about how the organizations that received a funding recommendation greater
than requested will incorporate the additional funds into their submitted proposals. The
proposals by the organizations explained how the grant funding would be used to achieve
specific goals. The Commission's recommendation for additional funding above the requested
amount could present a scenario whereby additional funding would be given for uses that were
unidentified and may be unplanned for as part of the applicant's request. It is within the City
Council's purview and authority to make adjustments to the Commission's recommendation and
fund applicants at the amount requested.
In addition, at the October 20, 2015 meeting, the City Council provided direction to Staff
stipulating that grant funds should not be awarded to art and school based programs or events,
but be limited to critical human services such as health care, food and nutrition, and shelter.
The table below summarizes Staff's initial recommendation and the final recommendation from
the Commission for funding from the General Fund.

Affordable Housing Fund
The Human Services Commission and Staff recommended one organization for funding through
the City's Affordable Housing Fund. Housing services are allowable expenses per the City's
Zoning Ordinance, Section 8.683080A. These programs have been supported through this
funding source in prior years. Using the Affordable Housing Fund for these programs provides
additional flexibility in awarding funding to applicants and furthers the Affordable Housing
program goals.

Final grant allocations to all organizations will be determined based on available funding as
identified during the City' Council's adoption of the next two -year budget, and funding would be
available on July 1, 2016, with the necessary documentation provided. If reductions are
necessary, Staff will work with the organizations to make prorated reductions that comply with
the minimum grant threshold of $3,500 and any applicable CDBG regulations.
Attachment 4 is a resolution providing funding for all organizations except DPIE. Attachment 5 is
a separate resolution providing funding for DPIE only, to address Councilmember Biddle's
conflict of interest.
NOTICING REQUIREMENTS /PUBLIC OUTREACH:
A copy of this Staff Report was sent to the Human Services Commission and all grant
applicants.

On annual basis, the Human Services Commission reviews Human Services Grants Program applications for
the upcoming Fiscal Year. The Commission will receive presentations from community organizations
applying for grant program funding; discuss the grant applications; review Staff's recommendation for
funding levels, and make funding recommendations for City Council consideration at the February 16, 2016
meeting.
FINANCIAL IMPACT:
The Human Services Grants Program is supported by several funding sources including the Community
Development Block Grant (CDBG) Fund, the General Fund and the Affordable Housing Fund as
appropriate. The preliminary Fiscal Year 2016 -2017 CDBG allocation is $93,658. A portion of this funding,
approximately $72,709, is available for direct support of social service programs in the community. Final
allocations from the Department of Housing and Urban Development (HUD) will not be known until at least
March 2016. The Fiscal Year 2016 -2017 Human Services Support Grant funding cap is $130,000 (General
Fund) as established by the City Council. In compliance with grant program guidelines, all grants applications
must comply with a $3,500 minimum and $25,000 maximum per application (pro)ect) request.
RECOMMENDATION:
Receive organization presentations, review Staff's recommendation on funding levels and make funding
recommendations for City Council consideration.
DESCRIPTION:
The City of Dublin Human Services Grants Program provides financial support to local nonprofit
organizations serving the Dublin community. The program is supported through several funding sources
including the federal Community Development Block Grant (CDBG) program and the City's General Fund
and Affordable Housing Fund (as appropriate). Each year the City Council awards funding to organizations

that best meet the community's needs in support of the City's adopted Mission, Vision and Values
(Attachment 3) and needs identified in the Tri -Valley Needs Assessment (Attachment 4).
Prior to the Fiscal Year 2014 -2015 funding cycle, a City Council Grants Program Subcommittee provided
policy and grant funding recommendations to the full City Council for consideration. For Fiscal Year 2014-
2015, the grant process was modified to include a review of grant applications by the newly formed Human
Services Commission. The Human Services Commission assumed the role of reviewing funding applications,
receiving presentations from each grant applicant and making recommendations to the City Council. The
City Council will consider the Human Services Commission's funding recommendations at the meeting on
February 16, 2016 for inclusion in the Fiscal Year 2016 -2017 budget. Final consideration of the City
Council's grant funding recommendations is tentatively scheduled for May 2016 as part of the biennial budget
hearing.
Staff has reviewed the Fiscal Year 2016 -2017 grant applications to verify that all requests qualify for funding
based on the adopted program guidelines. As part of the review, Staff applied the City Council- approved
Rating Criteria (Attachment 5) to each grant application. Each funding source does have some variation in
application and use requirements; these differences were taken into consideration during the Staff review
process. The rating for each application can be found in Attachment 2.
The Rating Criteria is only one of several factors used in determining Staff's funding recommendations.
Other factors include consideration such elements as: the number of Dublin residents served and cost per
participant; past performance for existing Dublin programs; whether the project addresses one or more of the
14 Key Areas of Concern identified in the 2011 Tri -Valley Needs Assessment Report (Attachment 4); and
whether City funds are used to leverage other funding sources. Throughout the grant process, applicants are
regularly advised that the success or implementation of their program or project should not be solely
dependent on receipt of grant funding from the City of Dublin. Organizations are strongly encouraged to
leverage any City grant award with other revenue sources. Based on the Human Services Commission's work
in establishing local and regional priorities this past year, additional consideration was given to applications
addressing needs in the areas o£ Behavioral Health (mental health and substance abuse), Health Care
(including dental), and Senior Services.
Similar to past years, grant applicants were informed that the presentation order for this meeting would be
based on the date /time their completed application was submitted through ZoomGrants. Attachment 1
provides a chart listing the order in which applications were submitted and will be presented.
CDBG Program Background.•
Community Development Block Grant funds are provided by the U.S. Department of Housing and Urban
Development (HUD). The City of Dublin works with Alameda County's Urban CDBG Program to allocate
funds and with HUD to assure that all CDBG- funded activities are in compliance with federal regulations.
The City of Dublin has participated in Alameda County's Urban - County CDBG Program since 1982. The
City of Dublin, along with the cities of Albany, Emeryville, Newark, and Piedmont (cities with a population
of less than 50,000 residents), are called the "Urban County Cities," along with the unincorporated areas of
Alameda County. During the current Fiscal Year (FY 2015 - 2016), the City of Dublin received $93,658 in
CDBG funding. For FY 2016 -2017 Dublin's portion of funding is expected at the same amount; however,
only $72,709 of this amount is available for direct support to organizations.
Every year, the County dedicates a percentage of each Urban County City's total CDBG funding allocation to
the County of Alameda's Housing Rehabilitation Program. This Program is available to income qualified
households within each of the respective Urban County Cities as well as the unincorporated County for
needed home maintenance and repairs. In addition, the County may limit each Urban County City to only
12% of the CDBG allocation for service -based organizations and projects. The County's past practice has

been to review all applications received from each Urban County City to determine if the overall request for
service -based projects exceeds the 12% limit.
For the past few years, the County has requested that the City make adjustments to the service -based project
allocation as the County was projected to exceed the 12% limit. If adjustments are needed for Fiscal Year
2016 -2017, Staff will work with eligible grant applicants and the County to make adjustments as necessary,
including implementation of a prorated reduction for all CDBG awards, if necessary.
On May 6, 2014, the City Council approved a resolution authorizing the City to enter into a Public
Participating Jurisdiction Agreement (PPJ) with the Cities of Pleasanton, Livermore, and the County of
Alameda for repayment of a HUD loan to support Axis Community Health's Clinic Expansion). The City of
Dublin has been supporting predevelopment activities for this project over the past several years through the
CDBG program. The PPJ allows the agencies, with City of Pleasanton acting as lead agency, to borrow $1.25
million to support Axis Community Health's facility construction. As a result of the agreement, the City of
Dublin is required to repay its portion of the loan, a total of $265,618, (or $13,374.18 each year) through
CDBG funds. The funding allocation for repayment of the loan is identified as part of the Staff
recommendations in Attachment 2.
CDBG funds are categorized as federal funds with strict use, reporting and monitoring requirements to insure
compliance with federal regulations. Many organizations are not able to meet these requirements and are
therefore not eligible to receive CDBG funds. To comply with the County's CDBG program timelines and
to assist with timely reporting to HUD, the Commission's CDBG funding recommendations must be
considered at the City Council meeting on February 16, 2016.
Human Services Support Grant Program Background.•
Fiscal Year 2016 -2017 marks the 121h year of the City Council's adopted Human Services Grant funding
process. This formalized grant process was the result of the City Council's desire to standardize an application
and reporting process for organizations requesting and receiving financial support from the City's General
Fund and other City funding sources. The funding cap for Human Services Grants Program has been
established by the City Council at $130,000 from the General Fund. As appropriate, the General Fund
allocation has been supplemented by the Affordable Housing Fund. This approach has provided additional
flexibility in funding community organizations providing valued services to Dublin residents.
At the October 20, 2015 City Council Meeting, the City Council discussed the funding priorities for the
upcoming grants cycle. The discussion resulted in City Council direction that the Grants Program emphasize
funding programs specifically directed towards human service needs. As such the City Council decided that,
for Fiscal Year 2016 -2017 and beyond, grants would not be awarded to arts or school -based programming or
events, but would be focused on the most critical human services needs such as health care, food and
nutrition, homelessness, child care, etc.
Outreach Efforts
As in past years, there were many points of contact with organizations as part of the Human Services Grants
Program application process. These outreach and education efforts included:
• In October 2015, a letter was sent to all past grant applicants to notify of a change of Department
administration and of City Council's direction that the program would focus heavily on critical human
service needs.
• In October 2015, notices of funding availability and upcoming community workshops were mailed to
organizations.

• In October and November 2015, a Request for Proposal was advertised three times in the local
newspaper for Grants, with an open application period set between November 12 and December 11,
2015.
• Program announcements and information were posted on the City website with links to the Zoom
Grants Application.
• On November 12 in the morning and November 18, 2015 in the evening, City Staff held Mandatory
Grant Application Workshops with interested community organizations to review the online Zoom
Grants Application process and discuss program guidelines for the year. All Fiscal Year 2016 -2017
grant applicants attended at least one of the mandatory meetings.
• In January 2016, Human Services Grants Program information was distributed to the Human Services
Commissioners to familiarize them with the grant applications and online grant application program.
Analysis
During this year's application cycle, the City of Dublin received 21 applications from 18 organizations. Of
the 21 eligible applications received, seven (7) met the CDBG criteria and the remaining were considered for
other grant program criteria. Three (3) organizations submitted applications for two (2) different
programs /projects: Chabot -Las Positas Community College District /Tri Valley One Stop Career Center, Tri-
Valley Haven, and YMCA of the East Bay Tri -Valley Branch. When more than one application is submitted,
organizations are required to rank their funding requests based on the organization's priority. Upon Staff
review, all applications were deemed complete and all organizations met the minimum application
requirements.
Staff reviewed all applications and rated them according to the following City Council approved criteria:
• Organization and Management — Evaluates the agency's ability to run and manage its operations;
• Needs and Benefits — Assesses if the request meets the targeted population and meets at least one goal
and /or policy established by the City's Mission, Vision and Values or HUD guidelines;
• Funding and Budget — Evaluates the agency's ability to leverage funds, reviews fees and current audits.
Attachment 2 summarizes the Fiscal Year 2016 -2017 funding requests including: the organization's name and
program; the Fiscal Year 2015 -2016 funding award (if any); the organization's Fiscal Year 2016 -2017 funding
request; Staff s funding recommendation (including funding source); the rating score for each application; and
the number of "Needs" the service or project meets as outlined in the Tri -Valley Needs Assessment. The
Staff recommendations are based on the rating criteria and the additional factors outlined earlier in the report.
Finally, the program criteria provide that organization funding requests and awards must be between $3,500
and $25,000.
Community Development Block Grant (CDBG) Funds
Of the 21 eligible applications received during this year's application cycle, seven (7) applicants qualified and
indicated they were willing to meet the strict federal guidelines for use of CDBG funds.
Staff has reviewed past program performance and proposed activities of the service providers to ensure the
activities are in compliance with HUD /CDBG guidelines. As a requirement of funding, the City performs an
annual site - monitoring visit to each agency. This site - monitoring visit includes a review of organization and
client files to ensure compliance with HUD guidelines. There were no findings issued during the prior
monitoring period for any of the organizations funded through the CDBG program.

In the table below, of the anticipated $93,658 allocated to the City, $72,709 is expected to be available for
funding organizations through the Human Services Grants Program. Staff recommends funding six (6)
organizations for Fiscal Year 2016 -2017 through the CDBG program as follows; the one remaining
organization is recommended for funding through other sources (Community Resources for Independent
Living) :

These recommendations are based on City Council discussions, funding decisions approved during the last
several grant funding cycles, and the factors identified above. For the past six years, the City Council has
emphasized basic life needs such as food, shelter, medical services, and education in grant funding decisions.
The CDBG recommendations are reflective of that approach.
In addition to the CDBG funding recommendation of $72,709 in the table above, the City has three (3)
additional CDBG funding commitments which total the $93,658 in funding for the year:
• The County of Alameda administers the City of Dublin's rehabilitation program. In Fiscal Year 2016-
2017, $14,349 is preliminarily allocated to that program;
• Administration fees in the amount of $6,600 are allocated to support administration of the CDBG
program.
• An additional percentage is also used to fund County of Alameda administered tenant /landlord and
fair housing services for the City.
After funding the program's mandatory functions listed above, $72,709 remains for direct allocation to
qualifying applicants. Should there be further decreases in the CDBG allocation, Staff will apply the
percentage of the decrease evenly across all the organizations (with allowance for rounding and compliance
with grant guidelines). If adjustments are necessary to comply with the 12% service -based project cap, Staff
will work with the organizations and the County to bring grant allocations into compliance.
The organizations, as recommended, meet the CDBG program's national objectives of benefiting income
qualified persons, homeless persons or persons with disabilities; and all qualify as eligible uses of CDBG
funds. If approved, the City would enter into mandatory contracts with the County and with the funded
organizations. As required by CDBG guidelines, organizations would be required to submit quarterly reports
on their progress in reaching the goals and participate in annual site monitoring.

General Fund
Of the 21 eligible applications received during this year's application cycle, most applications met Human
Services Grant funding criteria. Staff has reviewed past program performance and proposed applicant
activities to ensure compliance with program guidelines. As a condition of funding, the City will require a
semi - annual and an annual summary report from each agency receiving funding. These reports detail how
many residents were assisted; how the funds were used; if the program goals as outlined in the grant
application were met; and the overall impact of the funding. Once submitted at the end of the calendar year
and Fiscal Year, the summary reports are provided to the Human Services Commission as an informational
item.
Axis Community Health submitted an application to request funds for pre - construction costs for
development of a dental care facility. This type of project request is not eligible for CDBG funding as a
service or capital project. As a result, Staff is recommending that the Commission consider funding this
project using the $30,000 the City Council set aside for the Commission to fund a one -time special project.

Affordable Housing Fund
Of the 21 applications received, one (1) organization is being recommended for funding through the City's
Affordable Housing Fund: Community Resources for Independent Living. Housing services are allowable
expenses per the City's Zoning Ordinance, Section 8.68.080A as long as income verification is collected.
These programs have been supported through this City funding source in prior years. Using the Affordable
Housing Fund for these programs provides additional flexibility in awarding funding for applicants furthering

The City of Dublin Human Services Grants Program provides financial support to local not - for - profit organizations serving Dublin residents
through designated funding sources. Each year the Dublin City Council awards funding to organizations that best meet the community's
human services needs in keeping with program guidelines and as outlined in the 2011 Tri- Valley Human Services Needs Assessment.
The Dublin City Council, at their October 20, 2015 meeting, provided direction that the grants program emphasize funding programs
specifically directed towards human service needs. As such, the City Council decided that going forward, grants will not be awarded to arts
or educational programs, but will be focused on the most critical human services needs such as health care, food and nutrition,
homelessness, child care, etc.
The Human Services Grants Program includes a competitive application process, mandatory participation in meetings and presentations to
the Human Services Commission, and strict financial and other reporting requirements.
TYPICAL FUNDING SOURCES
-General Fund
-Affordable Housing Fund
-Community Development Block Grant (CDBG)
Recipients receiving General Fund and Affordable Housing Fund grants are required to report activity on a mid -year and year -end basis
HUD CDBG fund recipients are responsible for reporting activity quarterly and verifying income of those benefiting from funding. More
specifically, CDBG requires that residents benefiting from funding must be at or below 80% of Area Median Income (AMI).
Income requirements can be found at the City of Dublin website at www.dublin.ca.gov /housing /.
Specific reporting requirements and any additional responsibilities will be included in the contracts that accompany all grants awards. In
addition, recipients must register their agency with 2 -1 -1, as appropriate.
GRANT FUNDING AMOUNTS
The Dublin City Council has established the following funding criteria
Minimum application amount: $3,500
Maximum application amount: $25,000
General Fund: The City of Dublin allocates up to approximately $130,000 annually for this grant program (based on funding availability). The
City of Dublin's funding for this program is not guaranteed and ultimately subject to City Council approval.
CDBG: The City of Dublin may receive Federal CDBG funds from Alameda County as part of a coalition of cities. Available funding for this
funding cycle is estimated to be approximately $72,000. Funds are not guaranteed and typically decrease from the initial estimate due to
adjusted federal funding allocations. " Please note: Alameda County has recently advised staff that funding for CDBG eligible applicants
may decrease for FY 16 -17; amount is currently unknown. Changes to the program will be communicated once known. You are encouraged
to apply for funding through this process. "
CDBG REQUIREMENTS
An organization receiving CDBG funding must collect the necessary income and household data from participants that receive services. At
least 75% of participants must be at or below 80% AMI. All CDBG subrecipients must obtain a DUNS number and register with System for
Award Management (SAM). If your organization or agency has questions as to whether or not they may be eligible to receive CDBG funds,
please call the Parks and Community Services Department at (925) 833 -6645.
The United States Housing and Urban Development Department (HUD) has required that all CDBG grantees develop a Limited English
Proficiency /Language Access Plan (LEP /LAP) to determine language translation needs for limited English speaking residents to encourage
access to services and housing. As a result, agencies must now provide detailed information on Limited - English populations served and
language access accommodations and outreach for Limited English speaking clients.

INSTRUCTIONS TO COMPLETE THIS APPLICATION:
1. The City will ONLY accept applications submitted on -line through the ZoomGrants system. The City will not accept hard copy, fax, or
emailed applications.
2. Applicants must attend a City of Dublin Grants Program Application Workshop. The dates /times are listed on our website at
www.dublin.ca.gov /grants. Attendance at one workshop is mandatory in order to apply for funding.
3. COMPLETE APPLICATIONS MUST BE SUBMITTED BEFORE 5:00 PM, FRIDAY, DECEMBER 11, 2015. At exactly 5:00 PM the system
will no longer allow you to save or submit your application. It is the responsibility of each applying agency to allow ample time to submit your
application before the deadline.
4. If your agency is requesting funding for more than one project or program, a separate electronic application must be submitted for each
project /program. Question #45 on the application will ask you to RANK the priority of funding for each program or project submitted. For
example, if an agency has multiple program /project applications (which may or may not be administered by the same person), those
program managers within the agency must communicate with each other to RANK the priority of funding for each application. There should
be only one #1, #2 etc.
5. You must indicate if you are requesting funding for Capital Improvements or Public Service. If you are requesting funds for both you must
submit separate applications for each funding type. The City of Dublin will determine which funding source best fits your project and provide
that recommendation to the City's Human Services Commission. Final awards will be granted by the City Council.
6. You are required to answer each application question. If a question does not apply, please answer with "N /A" or check the "Not
Applicable" box.
7. Please visit the City's website (www.dublin.ca.gov /grants) to download additional grant application supporting documents including the
Rating Criteria, Dublin's Mission, Vision, Values, Critical Dates, the 2011 Eastern Alameda County Needs Assessment Report and CDBG
Income Limits.
8. IMPORTANT NOTE: The ZoomGrants System will allow you to submit your application without attaching the REQUIRED documents;
however, if you fail to upload and submit the required documents, your application will be deemed "INCOMPLETE" and WILL NOT BE
CONSIDERED FOR FUNDING.
APPLICATION REVIEWAND FUNDING RECOMMENDATION PROCESS:
1. The City of Dublin Human Service Commission will review all funding proposals (tentatively) on January 28, 2016. All applicants must be
present at that time to present a brief 3- minute presentation on their funding request. Dublin Staff will evaluate all applications based on a set
of Rating Criteria which is posted on the City's web site at www.dublin.ca.gov /grants.
2. All agencies submitting an application are required to send a representative to this meeting who is knowledgeable about the application,
organization, and program and is able to answer questions from commission members. Responses will be evaluated as part of the funding
recommendation process.
3. Tentatively, Staff expects to present funding recommendations to the City Council on February 16, 2016, at 7:00 PM in the City Council
Chamber (100 Civic Plaza, Dublin).
4. If your agency is awarded Grant funding, you will receive an Agreement that must be executed prior to encumbering funds on July 1,
2016.

FISCAL YEAR 2016 -2017 HUMAN SERVICES GRANTS PROGRAM
WHEREAS, on January 28, 2016, the Human Services Commission recommended eighteen
(18) projects /programs receive grant funding through the Human Services Grants Program; and
WHEREAS, the Human Services Commission has provided the City Council with Fiscal Year
2016 -2017 Human Services Grants Program funding recommendations; and
WHEREAS, Exhibit A attached hereto and made a part of this Resolution, reflects the Fiscal
Year 2016 -2017 Human Services Grants Program funding recommendations (all funds) for the City
of Dublin; and
WHEREAS, the action taken by this resolution shall approve the funding recommendations of
all funds excluding the Dublin Partners in Education (DPIE) appropriation.
NOW, THEREFORE, BE IT RESOLVED that the City Council of the City of Dublin does
hereby take the following actions-
1 . Approve and adopt the Fiscal Year 2016 -2017 Funding Appropriation for the City
of Dublin Human Services Grants Program, as outlined in Exhibit A (Excludes DPIE
Appropriation);
2. Approve the allocations as recommended by the Human Services Commission;
3. Include the allocations in the Fiscal Year 2016 -2017 budget;
4. Direct staff to work with the County of Alameda as required on the CDBG funding
allocations; and
5. Authorize the City Manager to execute agreements with the County of Alameda for

January 28, 2016
A Special Meeting of the Dublin Human Services Commission was held on Thursday, January 28,
2016, in the City Council Chamber of the Dublin Civic Center. The meeting was called to order at
5:33 PM by Chair Strah.

On a motion by Vice Chair Brown, seconded by Cm. Fiedler, and by a vote of 5 -0 -0, the
Commission took the following action:

8.1 Election of Officers
Ms. Rhonda Franklin, Management Analyst, provided the specifics of the item as outlined in the
Staff Report regarding the election of officers for the Chairperson and Vice Chairperson
positions for the 2016 calendar year.
On a motion by Vice Chair Brown, seconded by Cm. Fiedler, and by a vote of 5 -0 -0, the
Commission took the following action:

Elected Chair Strah to remain as Chairperson fort he 2016 calendar year.
On a motion by Cm. Hassan, seconded by Cm. Fiedler, and by a vote of 5 -0 -0, the Commission
took the following action:
ACTION
Elected Vice Chair Brown to remain as Vice Chairperson for the 2016 calendar year.
8.2 City of Dublin Fiscal Year 2016 -2017 Human Services Grant Program Funding
Recommendations.
Ms. Franklin presented the Staff Report regarding the City of Dublin Human Services Grant
Program funding recommendations for Fiscal Year 2016 -2017.
Chair Strah briefed the meeting attendees on the presentation process and outlined the agenda
for the item.
Cm. Songey stated for the record: "I want to publically state that I am a Core Team Member of
CityServe of the Tri- Valley. I understand that this organization is seeking grant funding today.
As an unsalaried member of this nonprofit corporation I am permitted by law to participate in
decision making regarding this organization, but am required to disclose my affiliation. I would
ask that this statement [be] included in the minutes."
Cm. Fiedler stated for the record: "I wanted to publically state that I will recuse myself from
making and participating in making any decisions involving grant funding to Senior Support
Program of the Tri - Valley. I will do so because I currently serve as a Voting Director of the
organization. I understand that the Senior Support Program of the Tri - Valley is seeking funding
today and its application may eventually make its way to the City Council. I will remove myself
from the room when this organization presents to the Commission and will again remove
myself during Commission deliberation relating to this organization. I would ask that my
statement [be] included in the minutes."
Chair Strah stated for the record: "I wanted to publicly state that I will recuse myself from

Ms. Lara Calvert presented on behalf of Spectrum Community Services for the Meals on Wheels
for Dublin's Homebound Seniors program.
YMCA of the East Bay, Tri- Valley YMCA was called to present on their funding request in
support of Mental Health Services. A representative was not in attendance to present on
behalf of the organization.
Ms. Gloria Gregory presented on behalf of City Serve of the Tri - Valley for their organization.
YMCA of the East Bay, Tri - Valley YMCA was called to present on their funding request in
support of YMCA Day Camp — Affordable Summer Child Care. A representative was not in
attendance to present on behalf of the organization.
Chair Strah announced that the organization presentations had concluded and the Commission
would take a break to complete their individual funding recommendations and submit them to
Staff for input into a cumulative worksheet for discussion after the break. Chair Strah recessed
the meeting for a break at 7:24 PM.
Chair Strah called the meeting back to order at 8:39 PM.
Chair Strah re- opened the floor for public comments. Seeing none, Chair Strah closed the floor
for public comments.
Ms. Franklin reviewed the cumulative worksheet that was compiled from the Commissioner's
individual recommendations for each organization.
Cm. Fiedler recused himself from the meeting for discussion regarding Senior Support Program
of the Tri - Valley.
Chair Strah recused herself from the meeting for discussion regarding Dublin High School PFSO.
Ms. Franklin explained that eligibility for grant funding included this mandatory presentation
before the Commission, so as a result of not being in attendance at the meeting, YMCA of the
East Bay, Tri - Valley YMCA is disqualified from the process.
On a motion by Cm. Fiedler, seconded by Cm. Hassan, and by a vote of 5 -0 -0, the Commission
took the following action:
ACTION:
Recommend Human Services Grant Funding to the City Council as outlined in the following
table and to consider the Axis Community Health Capital Project for Dental Services for the
Commission's one -time special project fund at a future meeting.
