Sandwiched in this area are two IT technicians and all the server, phone and network equipment to support the IT operations of Axis Community Health. Individual Counseling "Rooms" Counseling areas are in close proximity to each other. Counseling areas are open cubicles which often times lead to clients and counselors whispering. Counseling areas are cramped. s October 27, 2008 Ms. Carol Beddome Development Director Axis Community health 4361 Railroad Ave Pleasanton CA 94566 cc: Mr. Rick Eastman, Director of Behavioral Health Services Subject: Proposal for Construction Service for tenant improvements to the combined Buildings: H and I for Drug and Alcohol Department. Thank you for meeting with me and allowing Antrim to assist Axis Community Health with their facility needs. Antrim has a long relationship with Axis when it was formally know as Valley Community Health, as well as building medical based facilities for numerous clients. In 1997 Antrim worked with your staff and designed and built your facility at 3922 Valley Ave. whereas Axis was also the tenant and Antrim the landlord. Antrim also worked with the staff and the BOD of the then "Valley Community Health" to build the Pediatric Wing on the south side of the parking lot on Railroad Ave. For this project, we understand that you want to improve the existing facility at buildings H and I to be more compatible for a larger Drug and Alcohol Department. The needs list we were instructed to consider from Rick Eastman and Carol Beddome are as follows: • Two rooms to house group sessions of approximately 10 people • One large room for group session of approximately 20 people. • Keep the existing IT room and enlarge it • Keep the existing restrooms • Make Five (5) private counselor rooms • One (1) staff office • A reception space • Allow for phasing of construction due to limited space to relocate staff To accomplish this requirement, Antrim offers the following proposal and budget for your consideration. This concept is intended to allow Axis to make decisions based on budgets available, how much money in addition to fund -raise for, or to allow only portions of the work to be done based on funds and staff relocation issues. We have attached an example of one possible floor plan solution that could work using the above needs list you gave us. It is similar to the sketch I provided for Rick on his white board. The attached sketch is a rough draft format only and we ask you to keep it as Antrim's confidential work product as it was used by us and our team to offer a base budget for your consideration. 1635 CHESTNUT STREET, SUITE A, LIVERMORE CA 94551 PHONE (925) 426 -2444 FAX (925) 426 -0371 Antrim is a design build contractor, whereas we employ outside architectural services to complete the City permit process related architectural work, after the scope of the project finalized. We propose this method for non- profit based firms where budgets are not yet established, however a majority of cost lies within the construction and not the design fee. To accomplish this, Antrim proposes that we work a program in reverse from what one would typically do, whereas we start with your budget, then offer a design that fits the budget with the knowledge of the needs, rather than spend funds on a full design then wait for an accurate construction cost based on drawings, only to find out at the 11`h hour that the cost for construction is more than the budget can allow causing re -work, delays, and additional design fee expense, thus eroding the total funding package. The process will move forward as follows: 1. Design Development. This process is where Antrim sketches 2 floor plan layouts for your consideration after it measures the facility and confirms the needs of Axis. The sketches are enough to be able to estimate a budget price that is within 10 %, plus allows for decisions to be made by Axis for scheduling of staff movement while construction occurs. One draft example is presented as an attachment. This cost is nominal and only reimburses us for our time. 2. Construction Documents Phase: Once Axis chooses a floor plan, then Antrim hires its team including design build subcontractors to complete a set of documents adequate to receive a permit from the City of Pleasanton. As well, the construction portion of scope and price is reestablished. 3. Construction Work and Scope. Based on the budget, the scope will morph to include or exclude specific items that can be paid for now, deferred, added or deleted. Pricing: 1. Antrim will perform Design Development for $1,500. This includes meetings with Antrim staff and Axis staff and providing 2 floor plans, whereas Axis will choose one and Antrim will scope and price it. 2. Antrim can offer a fixed fee for a permit set of documents regardless of the version selected in 1 above, since total square feet of the building is existing and not subject to change for this project. The price for construction documents (CD's) is $9,800. The only exception for a price difference is if new restrooms are required, concealed conditions are revealed, or exterior construction to the building occurs. The work included in the CD phase is architect's drawings, details for walls, a simple finish schedule indicating type only, a reflected ceiling plan, electrical outlet and lighting plan, and HVAC re -duct distribution plan. 1635 CHESTNUT STREET, SUITE A, LIVERMORE CA 94551 2 PHONE (925) 426 -2444 FAX (925) 426 -0371 ) ■ 3. Construction budget based on the floor plan sketch shown is expected to be in the range from $85,000 to $110,000. The construction scope of work includes: * An allowance of City permits of $2,500. * Removal of walls as shown on sketch of 10- 22 -08. * Removal of floor coverings. * Creation of 14 areas: Hallway, reception, 5 counselor rooms, 1 staff room 2 small group rooms, 1 large group room, and the existing I.T. room & 2 restroom. * Installation of 150 linear feet of new walls, painted and insulated * Paint existing walls on interior * New VCT (vinyl colored tiles) in hallways, reception, and the 3 larger group rooms. * Carpet in 5 counselor rooms. Allow $20 /yard carpet materials * Vinyl base cove at all walls * New acoustical ceiling in all rooms but bathroom and IT room. Standard T -grid with flat acoustical panels. * Eleven (11) new interior doors with knock down type metal anodized frames, no locks (note: needed as existing doors are wrong swing and size). * Redistribute HVAC ducting and relocate thermostat for new room layout * Allow for 40 new 2 x 4 troffer flat lens prismatic lights. * Allow for 30 wall outlets, location TBD * Allow for 13 new switch locations. Excluded is: Bathroom work, IT room work except to remove portion of wall and add door location, Phone or data wiring or work, exterior doors, windows or work outside, phasing of work or remobilization, new HVAC unit except to re -duct, window frame work for wall abutting at exterior wall, alarms, security, interior signage or directory, electrical panel upgrades, fire sprinklers.
