AGENDA STATEMENT
CITY COUNCIL MEETING DATE: May 3, 2005
SUBJECT: Presentation by the Alameda County Health Care Service Agency
regarding"Tri-Valley Initiative to Expand Adolescent Health and
Wellness Services".

RECOMMENDATION: ' 1) Receive presentation
2) Determine if City should participate in the Tri-Valley Health
Care Initiative
3) Authorize Staff to advertise for interested youth participants
FINANCIAL STATEMENT: Staff time would be required to participate on the Steering
Committee. Any additional costs to the City are not known at this
time.
DESCRIPTION: . Alameda County Supervisor Scott Haggerty is requesting
participation of the cities of Dublin, Livermore and Pleasanton with Alameda County in the Tri-Valley
Adolescent Health Initiative. The purpose of this initiative is to improve access to and expand health and
wellness services for adolescents living in the Tri-Valley. -
This initiative as currently envisioned has four(4)major objectives.
1. 'Conduct assessment of youth and community needs.
2. Increase access and utilization of adolescent health and wellness services.
3. Build an adolescent health and wellness facility in the Tri-Valley. '
4. Build youth leadership and development.
The County has requested that the City appoint three (3)youth representatives (ages 13-24)to a Youth
Planning Board for a two-year term. The County has also requested that the City Manager or his designee
participate on an Adult Steering Committee.
If the City Council is interested in participating in this effort, Staff would recommend that the City
advertise for interested youth to serve on the Youth Planning Board. The County plans to make a
presentation to the City's Youth Advisory Committee in the near future to inform them of this initiative.

Trl-Valley Initiative to Expand Adolescent
Health &Wellness Services
Background
In Alameda County, adolescents are the least likely of any age group to access traditional health
services. To address this growing need, Alameda County Supervisor Scott Haggerty, the Cities
of Dublin, Livermore and Pleasanton, and the Alameda County Health Care Services Agency
(HCSA) have partnered to launch the Tn- Valley Adolescent Health Initiative to improve access
to and expand health and wellness services for adolescents living in the Tn-Valley region_
The partnership's vision is to support the positive growth of all youth in Eastern Alameda
County toward achieving overall health and well being, educational and career success. Using a
youth development and community participation approach, this regional initiative will
implement a collaborative planning process to build a youth services facility that addresses the
diverse needs of the region's rapidly growing population of children and youth. One of the main
goals of the youth services facility is to increase access to comprehensive medical care, mental
health, and health education, and to promote health and wellness in a streamlined and
coordinated effort.

This regional collaborative effort to maximize and expand adolescent health and wellness
services for youth and young adults in Tn-Valley has four major objectives:

Conduct Assessment of Youth & Community Needs
The Initiative will include a regional community needs assessment using a collaborative, youth-
driven participatory research model to assess the health needs and service gaps for adolescents
and identify program. recommendations.

Key Activities
Å Form a regional Steering Committee with. Tn-Valley youth, community organization
representatives, city staff, Health Care Services Agency staff and Board of Supervisor staff
that meets on a quarterly basis to build ongoing support and identify resources and funding to
address the project's start-up and long-term sustainability needs.

B. Create a Youth Planning Board (YPB) made up of youth ITom throughout the region to
engage other youth in the needs assessment and planning process, help identify priorities and
make program recommendations. .
C. Conduct a quantitative and qualitative assessment of the region's youth demographic, health
outcomes, resource needs, and service gaps.
D. Survey existing health and wellness services and programs provided by county, city, school
and community providers for youth in the Tn-Valley region.
Increase Access & Uflllzaflon of Adolescent Health & Weliness Services
Guided by the community nëeds assessment, the Steering Committee wiÏ1 engage ina
collaborative planning process to evaluate ways to increase access to and improve the delivery of
critical health and wellness services for youth and young adults.
Key Acflvlfles
A. Evaluate current service delivery models used by providers throughout Tri-Valley.
B. Develop strategies to increase access to and stream1ine the delivery of existing health and
wellness services for youth and young adults.
C. Create opportunities for county, city, school and community providers to collaborate and
address common service provision issues and goals.
D. Identify opportunities to leverage county, city, school and community resources and
programs focused on service provision to adolescents.
Build an Adolesc~_r1t Hel:lth & Wellness Servlc:;es Facility
The Stooring Committee will playa central role in leading a planning effort to build a regional
adolescent service facility to provide high quality, comprehensive health and wellness services to
youth and young adults in Tri-Valley.

Key Acflvltles
A. Develop a fund· development and sustainability plan to raise the one-time capital
improvement fund and ensure the long-term viability of the regional adolescent services
facility
B. Work with city and county representatives to identify and evaluate potential sites to develop
the facility.
C. Involve youth, community and service providers in the planning and design of the facility to
create a youth-centered environment.
D. Formalize commitments with county, city, school, and community partners and stakeholders
to holster long-term political and financial support
E. Engage in marketing and branding efforts to build community and regional support for the
adolescent youth facility.
Build Youth Leadership & Development
Throughout the planning and implementation of the previous three areas, youth ITom the Tri-
Valley region will be engaged to foster the development of youth leadership.

The Alameda County Health Care Services Agency (HCSA), through its School-BasedlLinked Health
Center Coalition (SBLHC) provides age-appropriate, comprehensive health and wellness seIVices to
adolescents through an integrated service delivery model. The goal of SBLHC Coalition is to improve
health care access, health care utilization and health status of adolescents.
